Weight loss medications
- Severe, persistent stomach pain
- Severe means:
- It’s so intense (around 8 or more out of 10) that you can’t focus on anything else
- It makes it nearly impossible to handle normal daily tasks (like walking, getting dressed, or eating)
- It doesn’t improve with over-the-counter pain relief or rest
- It radiates to your back or underneath your right ribcage or your right shoulder
- You also have severe vomiting (3 or more times a day, or having coffee grounds or blood in vomit)
- These symptoms could indicate pancreatitis, a serious condition that requires urgent medical attention.
- Severe means:
- Difficulty breathing, or swelling of the lips, tongue, or throat
- These could be signs of a severe allergic reaction, which can be life-threatening.
- Severe pain underneath your right ribcage that may spread to your shoulder or back
- This may be accompanied by vomiting, a high temperature or a rapid heartbeat.
- These symptoms could suggest acute gallbladder problems.
Mounjaro:
- If you miss a dose, take it as soon as possible within 4 days. If more than 4 days have passed, skip the missed dose and take your next dose on the usual day.
- Never take a double dose to make up for a missed dose.
- If you’ve missed multiple doses, contact us to discuss restarting your treatment.
Wegovy:
- If you miss a dose, take it as soon as possible within 5 days. If more than 5 days have passed, skip the missed dose and take your next dose on the usual day.
- Never take a double dose to make up for a missed dose.

It is important to remember that weight loss with medications like Wegovy and Mounjaro is a gradual process.
These medications begin working soon after your first dose by reducing appetite and regulating blood sugar levels. They stay in the body for about a week, which is why they are taken once weekly.
While some people may experience weight loss within the first few weeks, Wegovy and Mounjaro are designed as long-term treatments for obesity, rather than quick fixes. Weight loss is more likely to be steady over many months, and everyone's journey will be different — so don’t worry if your results aren’t immediate.
Clinical trials show that you can lose up to 15–20% of your body weight, but this can take over a year.
Be sure to reassess your lifestyle to make sure you are in a calorie deficit. This is when you burn more calories than you consume, and is key for weight loss.
There’s no single “right” dose for everyone. Your clinician will work with you to find what feels best, balancing comfort, side effects, and results.
Some people stay on a lower dose for longer; others move up over time. The goal is always the same: to help you feel well, manage side effects, and see sustainable progress at a pace that suits you.

Some people taking GLP-1 medications, such as semaglutide (Wegovy) or tirzepatide (Mounjaro), may notice changes in their vaginal bleeding. This can happen more often if they are also using hormone replacement therapy (HRT). Although this is increasingly observed, official guidance is still limited.
These changes might happen because GLP-1 medications slow down digestion. This can affect how your body absorbs hormone treatments — especially HRT taken as tablets, particularly oral progestogens. There don’t appear to be the same issues with hormone patches or intrauterine devices (like the Mirena coil). Weight loss and other body changes may also play a role.
This kind of bleeding is usually not serious, but it’s important to tell your doctor if you experience new, heavy or ongoing bleeding. They can check for other causes and may suggest adjusting your HRT or carrying out further tests if needed.
If you’re switching from one GLP-1 weight loss treatment to a different one, you may be prescribed what’s called an “off-label dose”. All this means is that the dose the prescriber has chosen is not the officially approved starting dose.
The medication we prescribe is fully licensed in the UK. Off-label prescribing simply uses a licensed medication in a different way. Our prescribers will only issue an off-label dose when they think it’s safe and appropriate for you.
Why prescribe this way?
A prescriber might issue an off-label dose as it may help you:
- Continue your current weight loss progress when you switch medication type
- Avoid having to start again on a lower dose and slowly increase it over time
Important to note:
- There’s limited long-term data for this approach
- You might notice more side effects, such as nausea, vomiting, diarrhoea, or stomach discomfort
- Dose matching between GLP-1s is not exact, so be aware that different bodies can respond differently
